Overview
The ICU Nurse with Case Management experience is responsible for delivering high-quality critical care nursing while coordinating multidisciplinary care plans to support optimal patient outcomes and safe transitions of care. This dual-role combines bedside ICU clinical expertise with case management responsibilities to ensure continuity of care, patient advocacy, and collaboration with families and clinical teams.
Key Responsibilities
Clinical Responsibilities (ICU Nursing)
- Provide evidence-based critical care nursing to patients requiring advanced life support, ventilator management, and continuous monitoring.
- Perform comprehensive patient assessments and implement individualized nursing care plans.
- Administer medications, infusions, and therapies safely and accurately.
- Monitor and respond to changes in patient condition promptly using critical thinking and ICU protocols.
- Assist physicians during procedures and implement medical treatment plans.
- Maintain accurate clinical documentation in accordance with facility and regulatory standards.
- Comply with infection control protocols and patient safety standards at all times.
Case Management Responsibilities
Coordinate holistic patient care plans in collaboration with physicians, therapists, social workers, and family members.Support discharge planning and safe care transitions from ICU to step-down, long-term care, rehabilitation, or home care settings.Conduct family meetings to provide updates on clinical progression, discharge planning, and care goals.Advocate for patient needs and continuity of care throughout the care journey.Support utilization review and ensure appropriate documentation for insurance / authorization requirements.Identify barriers to discharge and proactively work to resolve them.Lead multidisciplinary discussions and care coordination huddles.Educate patients and families on disease conditions, treatment plans, and post-discharge care.Maintain professional and empathetic communication to support family expectations and clinical realities.Qualifications
Bachelor of Science in Nursing (BSN) or equivalent.Minimum 3–5 years of ICU / critical care nursing experience .Case management, discharge planning or care coordination experience required .Certification in Critical Care (CCRN) or Case Management (CCM) is an advantage.BLS and ACLS certifications are mandatory.Seniority level
